Submit a comment on the proposed CY2026 Physician Fee Schedule caregiver training rule.Each year, the Centers for Medicare and Medicaid Services (CMS) reviews payment practices and policy changes under fee-for-service, or “traditional,” Medicare. In recent years, caregiver advocates have made significant strides through policy wins that formally recognize family caregivers as vital members of the care team.
Caregiver Policy Wins: CY 2024 & CY 2025
CY 2024 Final Rule
CMS made a historic breakthrough by finalizing payment for Caregiver Training Services (CTS). New CPT codes allowed physicians, therapists, and other professionals to bill for caregiver training—even when the patient is not present. CMS also expanded the definition of “caregiver” and permitted multiple training sessions annually, embedding caregiver education into routine care.
CY 2025 Final Rule
CMS built on this progress by introducing new HCPCS G-codes for CTS, including direct care and behavior management training. Importantly, CMS extended CTS coverage to telehealth, broadening access and reinforcing caregivers’ role as essential partners in patient care.
What’s Proposed for CY 2026
In July 2025, CMS released its proposed CY 2026 Medicare Physician Fee Schedule (PFS), which includes further enhancements to caregiver training and services that address health‑related social needs (HRSNs)—a key opportunity to deepen policy progress with your input Centers for Medicare & Medicaid Services Centers for Medicare & Medicaid Services Centers for Medicare & Medicaid Services.
It is CRITICAL for CMS to hear from both individual caregivers and organizational advocates about these proposed amendments.
